Does Insurance cover Wellness Care?
Major medical insurance uses three main tools to identify and manage disease: simple lab tests, drugs and surgery. Wellness care, which is intended to develop independence from conventional care is not considered “standard” and is not covered.

Most insurance plans will typically pay for standard lab testing (unless you have a restrictive HMO, Medicare or Medicaid) and in most circumstances I can order them for you (even if you are out of state). If you don’t have insurance for initial lab testing then we have discounted testing by going direct to Labcorp, which makes good testing very reasonable.

In rare circumstances some insurance plans may reimburse a portion of costs related to periodic office visits and consults, so I recommend you submit invoices (which we can supply on request) after the fact just in case.

Should I get labs done first?
Under most circumstances I wait until we have done your Initial Consultation before we decide what tests are appropriate. The reason I do this is to make sure your Diagnostic Strategy is as individualized as possible. If you are traveling a distance to see me I would suggest you get your labs done ahead of time so we can review them right away. In essence we combine the first two sessions (Initial Consultation and the Report of Findings) into a single session, avoiding two trips. The trade-off here is that there may be additional testing that we might need and is relevant but we won’t know that until after your Initial Consultation.

What kind of testing will you recommend?
That depends entirely on the problems we are trying to understand and what we discover along the way. The very basic requirement is a set of comprehensive blood work, which is outlined below.

The basic labs we will need are:

  • CBC with Differential and Platelets
    1. Red and White blood cells
  • Comprehensive Metabolic Panel
    1. Glucose, HbA1c, LDH, Uric Acid
    2. Electrolytes (Sodium, Potassium, Chloride, Carbon Dioxide, Phosphorous, Magnesium)
    3. Liver and Gallbladder enzymes
  • Iron Panel
    1. Serum Iron, TIBC, Saturation % and Ferritin
  • Lipid Panel
    1. Total Cholesterol, HDL, LDL, Triglycerides,
  • Thyroid Panel
    1. TSH, Total T4, Total T3, T3 Uptake, FTI, Thyroid Antibodies
  • Vitamin D (25-OH)
  • Homocysteine
  • Fibrinogen

Beyond standard lab testing the most common “specialty” tests that could be relevant include

  • Adrenal Status (saliva)
  • Hormone Balance (saliva)
  • Gut-based testing
    1. Infections and probiotics (stool)
    2. Leaky Gut (serum)
    3. Gluten and gluten-related food sensitivities (serum)

What can I expect during the Initial Consultation?
The first thing you can expect is that I will shut up and LISTEN to you! The average doctor’s office visit across the US is only 7 minutes long. That’s barely enough time to say hello, let along explain what’s wrong and engage on some critical thinking to figure out the best course of action.  It’s a lot easier for conventional docs to whip out a prescription pad or a referral than to think through problems with such time constraints.

We will take up to an hour to go through the following critical elements:

  • I want to make sure I understand what your primary concerns are – your symptoms
  • Once I understand your symptoms, I want to hear your story.  You see your symptoms are on the surface.  What is really important is how these are affecting you as a person, including the roles you play in life. How does this impact your ability to perform as a person, a spouse, a parent or an employee?
  • I also want to know how you imagine life would be different if you could fix whatever it is going on
  • Then, and only then, will we talk about the general approach that gives the best results
  • And how to construct an individualized Program that has the greatest potential for success

As long as we can come to an agreement about what to do and how to do it, we can get started right away.
